An employee was picking up a 10-inch, 230-pound piece of C-channel, using a 7.5-ton overhead bridge crane. The C-channel slipped off the clamping device and fell approximately 7 feet onto the employee's right leg. The employee was hospitalized with lacerations to the right leg.

An employee was installing wood trim surrounding a skylight inside a home. While holding a piece of trim in it's desired location, his pneumatic nail gun discharged, sending a 3/4-inch pin nail into his left forearm.

An employee removed a safety valve on a trailer that unknowingly contained nitrogen gas. The gas released from the trailer and pushed the employee backward off a scaffold approximately 10 feet before they struck a garage door, resulting in a pelvic injury.

An employee was sitting at her workstation when a nail gun malfunctioned and shot a nail into her left leg. She suffered a puncture and blood vessel injury and was hospitalized.

On November 25, 2023, an employee was using a high-pressured water hose to perform sewer line cleaning operations. The employee's right little finger and palm were lacerated by pressurized water from the hose.

An employee was hydroblasting when they slipped and the pressurized water stream lacerated their left ankle.

An employee was boarding a barge. As he stepped onto the gangway, it shifted and the handrail he was using collapsed, causing him to fall and strike his right knee against the gangway. His patellar tendon was torn.

An employee was inspecting the tension of a belt on the motor of a labeler machine when his right index finger got caught in a pulley. The employee's fingertip was amputated.

An employee was feeding stainless steel (2 x 6) sheet metal into a shear machine inside the warehouse. The pressure plate on the shear machine crushed the employee's right index fingertip. The employee sustained an amputation of the fingertip pad.

An employee was unpackaging fuel samples onto a table when they started having an allergic reaction resulting in facial swelling, red/itchy skin, hives, and respiratory symptoms. They were hospitalized.

An employee was advancing a sheet of foil through the knife on a pouch packaging machine when the knife cut his left index finger. The employee sustained an approximately 4-millimeter fingertip amputation.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
