An employee was on a 6-foot A-frame ladder while repairing the passenger-side marker light on a route truck. The employee lost their balance and fell to the ground below, sustaining fractures to their left wrist and L1 vertebra.

An employee was opening a liftgate by pushing a slider from the side. The employee's right index finger was pinched by the slider, resulting in an amputation to the finger just below the nail bed.

The employee slipped and as he went to catch himself, he accidentally engaged the up-down control for the forks. This caused the forks to fall onto his foot. The employee's right foot was crushed and all of his right toes were amputated.

An employee was working on a scissor lift when a portion of the unit came down and struck their head at the lift base. The employee sustained a head laceration and a skull fracture.

While attending the slab line, an employee observed that dough was accumulating on the plastic blades. He proceeded to clean the blades. His left index finger came into contact with an adjacent blade that moves laterally (left to right), resulting in a partial amputation.

A sanitation employee entered the chemical room and slipped on the wet floor. When he slipped, he kicked over a bucket containing cleaning chemicals. The chemicals splashed onto the employee's clothing and burned his skin.

An employee's right middle fingertip was caught between a chain and a sprocket in a machine. The fingertip was amputated. The machine was unguarded at the time.

An employee had just finished unloading material from a truck. He was descending using the truck's built-in ladder when he missed the last rung, fell to the ground, and broke his hip and left wrist.

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a gas tugger to lift metal sheeting. His left hand was pulled into the pulley, which crushed his left thumb, resulting in avulsions and other tissue damage. He was hospitalized and required surgery.
