An employee was moving a piece of an I-beam for welding when it rolled off the cribbing. The employee went to catch it when it fell and crushed the employee's finger, resulting in a partial amputation.

An employee was helping to disassemble large reels used to store steel wire when a 264-pound flange from the reel fell over onto the employee's left leg. The employee sustained multiple fractures to their leg and ligament injuries to the lower leg.

The injured employee was putting away materials on the warehouse floor as a co-worker was pushing a pallet onto a nearby shelf. The pallet knocked another pallet forward, causing it to fall onto the injured employee's shoulders. The employee suffered fractures in their right and left knees and ankles.

Employees were moving a single man lift into a building and reclining the lift to position it to fit through the door. The lift shifted and fell, causing the employee to sustain fractures to the left tibia and fibula. The employee was hospitalized and had surgery.

An employee was moving a light tower so it could be hitched to a truck. He grabbed the tongue of the trailer hitch on the light tower to slide it to the left. The tongue jack fell off, causing the light tower tongue to drop on the employee's right hand. The employee's middle fingertip was amputated.

An employee was serving as a racing official during a race on a dirt track. He was standing behind a guardrail when a race car lost control and came in his direction. He was struck by the car and sustained a broken radius, dislocated ulna, and a punctured lower right stomach.

An employee was moving an all-terrain vehicle (ATV) from storage to another location. The tire gripped the asphalt, causing the ATV to roll over. The employee was thrown to ground, resulting in rib and sternum fractures as well as a punctured lung. The employee was hospitalized.

An employee was helping with a vehicle on a race track. A race car struck the employee, who suffered an ankle injury. The employee was hospitalized.

An employee was placing a horse into a starting gate. The employee's left index finger was crushed between the horse and the metal gate. The fingertip was amputated.

An employee was standing on a piece of plastic that was on top of a piece of metal while a second employee was drilling up through the metal. The drill bit then punctured the first employee's right foot.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
