An employee was repairing a bucket loader in a maintenance shop. A jack kicked out, striking him in the head and knocking him to the floor. He suffered a skull fracture.

A temporary employee was riding a slow-moving truck when it hit a bump and he fell to the pavement, suffering a possible fractured skull, abrasions, and a cranial bleed.

An employee was riding on a trash truck when another vehicle failed to stop and crashed into the back of the trash truck. The employee's leg was pinned between the two vehicles, and she suffered a compound fracture that required hospitalization.

On April 8, 2021, an employee was walking on a warehouse floor when a forklift backed into him. He suffered foot and leg injuries and was hospitalized.

A truck driver had just dumped dirt from his truck on a shoulder. A skid steer loader began moving the dirt from the shoulder to grade out the slope. The skid steer was backing up to the slope to get its second load of dirt when the truck driver's right arm was caught between the back of the skid steer and the back of the truck. The driver's radial artery and radial nerve were severed and the driver was hospitalized, undergoing surgery.

An employee was pulling string on the side of a new road to be paved when a reversing tractor hit the employee. The employee suffered a head fracture.

An employee was operating the compaction levers on a truck when a motor vehicle crashed into the back of the stopped truck. The employee sustained fractured ribs and contusions.

A maintenance employee was moving a forklift from the loading dock when the forklift fell off the dock. The employee was struck by the overhead portion of the roll bar, injuring his right knee. He was hospitalized and underwent surgery.

An employee was collecting garbage when they suffered muscle cramping and were hospitalized due to dehydration.

An employee was climbing a truck-mounted ladder when the ladder broke, resulting in hospitalization with a fractured left leg.

An employee was standing on the driver side rear step of a trash truck while traveling down a road to collect residential trash. The employee went to signal the driver and slipped off the step while the truck was traveling 8-9 miles per hour. He fell 18 inches to the ground and was hospitalized with a fractured ankle that required surgery.

A crew was installing drilled micropiles alongside an outdoor covered deck foundation. The injured employee was working the front of the drill when a loose section of casing dropped onto the tip of a rig wrench and pulled the wrench down to an embedded casing. The employee's left little finger was caught between the rig wrench and the embedded casing and was amputated above the top knuckle.

Two employees were working to change a tire on a mobile slasher saw. The slasher weighs around 2,000 pounds and is towed. Employee 2 was lifting the saw while the injured employee was placing a block of wood under the frame for support. The slasher then lowered onto the block and the injured employee's left thumb was crushed between the slasher frame and the wood block, leading to an amputation at the first knuckle.

An employee was using a metal shear when it amputated the tip of his right index finger.

An employee was using a crane to move a 44-foot, 3,343-pound I-beam. As the beam was moving west, it straightened out (north to south), beginning to swing south toward the northwest corner of a building. The crane then sent the beam southward, directly toward the corner of the building. The beam caught the tips of the employee's left index, middle, and ring fingers against the building. The last joints of the middle and ring fingers were amputated, and the last joint of the index finger was partially crushed.

An employee was standing on a step stool, removing the nuts and bolts from the frame of a solar panel that was being replaced. The employee's cheek made contact with a connector with damaged insulation. The employee was shocked, briefly lost consciousness, and fell to the ground, suffering an injury to the left shoulder.
