On October 25, 2023, an employee was in the warehouse driving a forklift toward the dock door. The forklift struck a conveyor belt and the employee sustained broken ribs.
An employee was using a walkie-rider to load a trailer. As she turned a corner, the walkie-rider struck the trailer and the employee was thrown to the ground. She was hospitalized with a fractured left leg.
On March 26, 2025, at approximately 11:45 AM, an employee was operating a forming machine and sustained the amputation of their right middle finger. The machine was guarded at the time.
An employee was throwing garbage into a baler when they slipped, twisted their knee, and fell on their backside. The employee sustained a sprained left knee and was hospitalized.
On May 15, 2024, an employee was emptying a scrap hopper with a powered industrial vehicle (PIV) when their left hand was caught between the scrap hopper and the PIV fork. They sustained an amputation of the ring fingertip.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.
