The employee sustained facial injuries, broke two ribs and an arm, and also suffered a head injury. The employee was wearing a harness at the time but it was not connected to an anchor point.

On November 8, 2023, an employee was operating a stand-up pallet jack in the warehouse. As he was backing up, the pallet jack struck the basket of a parked aerial lift. His right ankle became caught between the pallet jack and the aerial lift basket resulting in a fractured ankle.

On October 26, 2023, at approximately 12:00 PM, an employee operating a four wheeler was on their way to an off-property break location. The employee stopped the four wheeler near the administrative office to allow a log truck to exit the property. The log truck backed up, and the employee's left ankle was caught between the log truck trailer and the four wheeler, resulting in a fracture and laceration.

An employee was being trained to use a counterbalance forklift. While operating the forklift under the supervision of his trainer, the employee exited one aisle and began turning into another when his left foot struck the end of a racking angle iron (rack protector). The employee was hospitalized with multiple foot and ankle fractures.

On April 7, 2025, an employee was operating a triple pallet jack when it slid and the employee's left leg was caught between the jack and a pallet rack system. The employee was hospitalized with a compound fracture to their left tibia.

An employee was operating a dock stocker when it struck an order picker that was stopped. The employee's left leg was pinned between the vehicles. The employee was hospitalized with multiple fractures and an avulsion to the foot.
