Final narrative
On January 22, 2017, an employee was driving a skid steer in an outdoor yard area where bulk materials are staged. At approximately 8:40 p.m., he exited the skid steer cab as a coworker was backing up an agricultural tractor/trailer. The employee was then struck by the tractor/trailer unit, fracturing several bones in his right foot.
An employee was looking into the bucket of a front-end loader to see how much material was in it. A skid steer backed into the employee, who then fell into the edge of the front loader's bucket and suffered internal bleeding in the abdominal area.
An employee was spotting for a forklift operation. The forklift backed over the employee, who suffered a complete amputation to one lower leg and a partial amputation to the other leg. The employee was hospitalized.
An employee was cleaning his work area and preparing it to begin making air-set molds
when a nearby forklift that was backing out from a load swung around and struck the employee. They sustained a lower left leg fracture.

An employee was driving a forklift to a shipping dock. While he was rounding a corner, a stack of totes fell, and the top of one of the totes struck the outside of his left calf. He was hospitalized.
A crew was preparing a 20-foot section of 12-inch high-density polyethylene (HDPE) pipe to be swung into a welder using an excavator, where it would be welded onto a longer run of pipe. One employee was facing the longer pipe in the welder while the 20-foot section was being rigged. The 20-foot section moved and struck the employee's left knee. The employee's left leg was broken below the knee.
The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.
An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.
A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.
