An employee was shifting a material support stand with a coworker when the employee's left thumb was pinched between the tops of the support being moved and an adjacent support. The distal tip of the employee's left thumb was amputated.

In preparation to cut rebar, a Bechtel Construction Services employee was picking up a battery-powered band saw from the ground with his right hand. He simultaneously reached underneath the housing of the band saw and grasped the blade with his gloved left hand. The band saw activated and caused a laceration to his left index finger.

An employee was emptying a self-tipping dumpster that was attached to the forks of a forklift. He tilted the forks forward to help facilitate the dumping of the dumpster. He then got out of the cab of the forklift, walked to the side of the forklift mast, and placed his left hand on the lower portion of the mast. He reached up with his right hand and disengaged the release lever on the back of the self-tipping dumpster, causing the dumpster to quickly shift forward. The resulting shift in weight of the dumpster caused the forklift forks to swing back into the mast, pinching the employee's ring finger between the forks and mast. The tip of his ring finger digit was amputated.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

Once the load became free and progressed upward, the employee's hand was caught and pulled into a snatch block. The employee's right thumb was broken and lacerated. The employee was hospitalized and required a surgical amputation.

An employee was operating a drill rig when when their hand got caught between a 1/4-inch steel cable and a drill rod, resulting in the amputation of four fingers.

An employee was overseeing the operation of a rope being respooled onto the drum of a wire winding machine. He went to remove a piece of wooden reel from the rope. His glove got snagged and his right hand was pulled into the roller, pinching his fingers between the machine's roller and the rope. The middle and index fingers were crushed.

An employee was visiting a patient's home to fit them with a wearable cardiac defibrillator. As he was walking from the front door to his car, a large dog broke free from a chain and attacked him. He was bitten on the forearm and fell. The patient was then able to restrain the dog. The employee was hospitalized with severe dog bites and required surgery.

An employee tripped over a junior I-beam about 4 feet long and 6 inches tall that was installed for circulating water valves and fell to the ground, resulting in a broken right-side rib.
