An employee was in the pump room troubleshooting a pump. He unscrewed the top panel of the pump and inadvertently hit the toggle switch that activated the pump. The internal belt caught his right index finger, causing a laceration that resulted in a partial surgical amputation.

On February 11, 2020, an employee was using a hose to spray off equipment. The employee attempted to move a conveyor belt, but its casters were locked; the employee's left hand slipped into the running conveyor belt, which amputated the middle fingertip.

Two employees were troubleshooting a conveyor in a feed mill. The conveyor caught one employee's coat and pulled his left arm in. The arm was lacerated and his index finger was partially amputated. He was hospitalized.

On October 29, 2018, at approximately 6:15 a.m., an employee was standing on the top step of the stairs connected to a truck cab when the step gave way. He fell to the ground, suffering three fractured ribs and a ruptured spleen.

On June 8, 2018, an employee was burned on the face, neck and back after opening a hot oil valve in the browning area.

An employee was stepping out of a car when they slipped and fell on ice, resulting in a femur fracture.

An employee was closing up for the weekend when they slipped on oil and brake fluid and fell. The employee suffered a leg fracture.

An employee was working in the forest flagging an area to be harvested. He took a step and his foot slipped on a stick hidden under the leaves, causing him to fall to the ground. He landed on his right foot/lower leg resulting in a fractured tibia.

An employee was inspecting an apartment to ensure it was ready for a new resident to move in. The carpet in the apartment had been recently cleaned and was still wet. As the employee went from the carpet to the tiled floor of the bathroom, they slipped and fell, resulting in a torn left hamstring.

An employee was delivering a letter along a rural carrier route when she stepped on an ice-covered snow drift, slipped, and fell to the ground. The employee sustained a right hip fracture that required surgery.

An employee was working on removing the gearbox from the drive shaft of the motor assembly of a feather drier. He was using a hydraulic tool to pull the pieces apart when a bolt dislodged from the eye of the gearbox and the corner of the gearbox broke off. The employee was struck in the nose (sinus cavity), resulting in hospitalization and surgery.

An employee was on a forklift checking the sprinkler system drain. The employee fell 8 feet from the forklift to the floor. The employee suffered a broken collar bone and was hospitalized.

An employee was working as a dumper on a packaging line. A passing forklift turned and pinned the employee against a guardrail. The employee suffered fractures to their ribs and a leg requiring hospitalization.

An employee was cleaning an upper mezzanine. When he leaned against the mezzanine gate, he fell through the gate opening and landed 11 feet below. The employee sustained fractured orbital bones.

An employee was assigned to clean a feed mill in the basement. He then rode a tool and material basket to return from the basement to the ground level. He fell out of the basket and landed about 5 feet below. The employee sustained fractures to his arm, lower back, and foot.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
