An employee's left ring fingertip was amputated while removing twine that was entangled in a roller. The employee's glove got entangled with the twine pulling the employee's finger into the feed roller. As the employee pulled the hand back, the fingertip was amputated by the twine.

An employee was reinstalling an idler machine. He went to tighten the bolt that holds the idler on and the machine started due to a sensor reading the movement. His right little finger was caught in the drive belt and the distal phalanx was amputated.

An employee was investigating a saw blade that was not spinning. The employee was checking the tension chain when the motor activated and his left index finger was caught in the chain belt, resulting in a partial traumatic transphalangeal amputation.

A piece of the board caught the employee's glove and pulled their right hand between the cutter head guard and the hold down. The middle and ring fingers were amputated at the first knuckle.

On November 9, 2023, an employee was removing plywood from the west face of a basement level buck hoist to prepare to dismantle one of the cabs. When the employee pulled on the plywood, it fell and pulled a rope that partially amputated the employee's thumb.

An employee was holding a rigging line and assisting in the removal of an old light pole from its base. When the load was lifted by a forklift, it shifted. The tag line lifted the employee off the ground and propelled him through the air. The employee landed on the ground and sustained back fractures.

The employee stepped on a metal grate that gave way and fell 8-10 feet. The employee was able to remove himself from the conveyor. His tibia was fractured just below the knee.

An employee was working to get a log back onto a carriage. The employee came into contact with the running saw, which amputated their arm.

While refilling a lubricant tank, an employee observed that a pump was not operating correctly. An employee was standing on a multi-purpose ladder installing head flashing over a window. Their feet were approximately 6 feet off the ground and three rungs from the top. The employee lost their grip on the drill they were using and the ladder tipped over. The employee fell from the ladder to the ground, resulting in a fractured left wrist, bruised chest, and lacerations to their face and lip.

An employee was inspecting the overhead area of a single-person lift and repositioning the lift through a double doorway. As they were transitioning through the doorway, they were caught between the doorframe and the vertical mast of the lift. The employee was hospitalized with injuries to the nose, left maxillary sinus, left orbital bone, and left jaw, with fractures at the maxillary sinus area.

An employee was loading a 4-inch wooden door into a pickup truck when he felt a pop in his right side behind his shoulder blade. He was hospitalized later that day and underwent surgery, having suffered a collapsed right lung.
