The employee was assembling a pneumatic activated butterfly valve assembly. He also attached the air source to it. As the employee was reinstalling the valve his right thumb was inside the valve assembly. The valve activated pinching the employee's right thumb tip resulting in amputation of the thumb tip including bone and the nail.
Employee's left-hand distal middle finger was crushed by a pallet jack. When injured, the employee was operating the pallet jack alone while moving an 1800-pound tote container.
