Final narrative
An employee was loading a bin with catfish feed when the machine clogged. The employee tried to clear the clog and caught their left hand in the rollers resulting in amputation of four fingers. The guard was removed at the time.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle.
