An employee was performing preventive maintenance on a conveyor and was wiping down the conveyor frame with a rag when the rag got caught between the conveyor belt and the roller. The employee's right hand was pulled into the machine and his right middle and index fingers were severely lacerated resulting in amputation. The conveyor was not locked/tagged out at the time.
An employee's foot was caught between an electric cart's accelerator and its brake as the employee stepped off the cart. The cart accelerated forward, striking a building column and breaking the employee's right leg/ankle.
