On March 23, 2017, at 4:30 AM, an employee was packing snacks at a packing table when the employee fell and struck an adjacent table, suffering head lacerations that required hospitalization.
An employee was walking behind the store with another associate to get paperwork from the delivery driver when she fell on the walkway and sustained a leg fracture that required surgery.
An employee was adjusting the forks on a forklift when a fork detached from the forklift. The employee jumped out of the way but fell on the concrete floor. The employee was hospitalized with a broken hip.
While attending the slab line, an employee observed that dough was accumulating on the plastic blades. He proceeded to clean the blades. His left index finger came into contact with an adjacent blade that moves laterally (left to right), resulting in a partial amputation.
