An employee was setting up a hydraulic pull tester station (5000 lbs. capacity). A plastic chain product was put under tension. While the employee was adjusting some locking washers with a spanner wrench, the plastic chain snapped and the employee's left middle fingertip was caught and amputated between the washer and a table.
An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.
