An employee was hooking a dolly to a trailer. The pintle hook receiver crushed and lacerated his right little finger, amputating the fingertip.

An employee was lowering the wheels on a tractor-trailer when the crank started to spin and struck him in the head. He suffered a concussion and was hospitalized.

An employee was raising the landing gear on a tractor trailer when the cranking handle for the landing gear hit and bruised his jaw.

On June 21, 2017,an employee was connecting two trailers with a converter dolly. The employee attempted to lift the dolly and suffered a back injury.

An employee bent over to lift up a handle to raise a dolly. When he came back up, he felt a pop in his back. He was hospitalized for a herniated disc in his back.

A crew was pulling a 4-inch plastic gas pipe off a reel and straightening it for installation. The injured employee stepped up on the trailer to cut the last band holding the pipe in the coil on the reel. As he turned to step off the trailer, the end of the pipe rotated and sprung out of the cage surrounding the coil, striking the employee on the side of the head and knocking him off of the trailer into the roadway. The employee suffered head trauma that required hospitalization.

An employee and a co-worker were performing a pick inside a clear well. They rigged the skid pan that was full of broken concrete. When lifting, the load began to swing toward a wall. The employee tried to stop the skid pan from swinging and was struck by the pan, resulting in fractures to their left hip and wrist.

The employee had just completed refueling a lattice crane that was breaking up material and was winding the fuel hose back into the fuel truck when he was struck by the catwalk/stairs of the crane. The employee was hospitalized with a laceration to his backside, possible internal bleeding, and a broken hip.

An employee was fusing 10-inch black rubber utility pipes together using a pipe fusion machine and could not get the pipes to set correctly. The employee used a nylon strap attached to an excavator to lift one side of a pipe off a steel plate. As his hand was between two pipes, the pipes came back together, partially amputating two of his fingers.

An employee was lifting four bags of a lime blend weighing approximately 10,000 pounds using a crane. The load swung and pinned the employee against a structural I-beam, resulting in fractures to the hip and pelvis.

An employee was inside a trailer, loading freight, when their leg was caught in shrink wrap. The employee fell, resulting in a fracture to their right thigh bone, which required surgery.

An employee was lowering a safe stack beam inside a trailer. The beam released, causing the employee to fall back and strike his head on the trailer floor. He was hospitalized with a brain bleed and a concussion.

An employee fell from a dock to ground level (less than 4 feet) and broke their tibia/knee.

On August 20, 2025, an employee was exiting a commercial tractor from the driver's side door when they fell to the ground, resulting in a fractured left ankle.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
