On June 12, 2025, an employee was removing a board from a cross-up on a landing table. While they were shifting the board onto a log loader, their right little finger got caught between the board and the lug. The employee suffered a fractured finger and a laceration of the knuckle that resulted in a fingertip amputation to the distal knuckle.

An employee was clearing a pile of woodchips from a planer feeder located in the dry finishing end when their left hand slipped and contacted a chip head resulting in amputation to the left little finger.

An employee was monitoring the flow of lumber into the edger and straightening cants/lumber for proper infeed when his hand was struck by a cant. The employee's left middle fingertip was amputated without bone loss.

An employee was grading lumber at the planer lug loader infeed when a broken piece of dimensional lumber was ejected from the planer trimmer and struck the employee. The employee sustained a concussion and intracranial bleeding.

An employee was doing maintenance at a sawmill. While getting ready to start the chip heads, he leaned in to observe a chain. The chain moved into position and caught the employee's right leg. He suffered a broken femur and was hospitalized requiring surgery.

An employee was tossing a damaged toilet into a compactor. The toilet bounced off the compactor and lacerated the employee's right wrist, cutting an artery and tendon.

An employee was operating a hydraulic jack to bend a frame when the jack came loose and struck the employee. They were hospitalized for a spleen injury.

A truck had been towed into the shop with a flat tire and a blown airbag. An employee checked the tire for damage and was airing the tire. The employee was installing the valve core when the bead slipped off the rim. Air escaped, and the aluminum wheel struck the employee's head and face. The employee became unresponsive and was bleeding.

On December 14, 2023, at 8:40 p.m., three employees were inspecting milling machines for startup. A few of the sleeves on the mill drum needed to be changed out. The injured employee was trying to input a sleeve on the mill drum using a hammer. The hammer hit the edge of the drum, which caused a metal piece to splinter off and strike the employee's cheek just below the left eye. The metal splinter embedded in the back of their eye socket, resulting in pain, swelling, and bleeding that required hospitalization.

An employee was pushing a line of carts into a vestibule when the door cable snapped and struck his right ankle. The employee suffered a fracture that required surgery and hospitalization.

An employee was walking down the off ramp from a box truck, carrying a door for delivery. A strong wind caught the door, causing the employee to lose balance and fall about 2 to 3 feet to the ground. He landed on his back and suffered a broken vertebra.

An employee was loading lumber from a rack into his truck by hand. He fell from the truck to the ground and sustained a fractured hip.

An employee was delivering lumber to a customer's job site. He was using a truck-mounted forklift to transport the material to the staging area. The surface of the path of travel changed from asphalt to soil and rock with a slight downward grade. When the forklift moved onto the softer surface, it sank into the ground and its center of gravity shifted forward, causing it to tip over. The employee's right arm sustained a closed, supracondylar humerus fracture.

An employee was helping two forklift operators move and inventory flat sheet metal that was staged on racks. The metal fell off one side and struck the employee's arms and abdomen. The employee sustained a laceration above the left eye, a left forearm injury, and a ruptured spleen. The employee was hospitalized and required surgery.

An employee was cleaning a press machine when their right ring finger became caught in the press, resulting in the amputation of the finger.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
