Final narrative
An employee was placing frozen fruit into the freezer section and was using a two-rung step ladder. When she stepped down onto the floor, her right leg gave way and she fell to the floor, breaking her right leg.
An employee was using a forklift to move pallets near a dumpster. While backing up, the forklift slid on ice and the employee's right hand was pinched between the dumpster and the roll cage, resulting in a partial degloving and broken bones. The injuries required surgery.
An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.
