On 6/11/17, at approximately 5:45 a.m., employee was operating a ride-on order picker in the mezzanine when he struck a pole. His left foot/lower leg was crushed and possibly fractured between the pole and the machine.

An employee was on a ladder, gathering materials from a shelf. She slipped and fell from the ladder to the concrete ground, suffering a broken heel and foot.
