An employee was framing a floor and attempted to push the floor assembly unit using a piece of wood at the end of a pusher. When the employee actuated the pusher, the employee's left hand was caught between the floor frame and the piece of lumber at the end of the pusher, resulting in an index fingertip amputation.

An employee was on the fourth rung of a ladder that was leaned up against a modular home. A tractor moved the modular home while the ladder was still leaned against it. The employee fell from the ladder, breaking a foot and hurting his/her back. The employee was hospitalized.

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.
