Three maintenance employees were working on chemical lines feeding into a large above ground wastewater treatment tank. Sodium hydroxide and sulfuric acid reacted in a floor drain and the chemicals sprayed onto the three employees. One employee was taken to the hospital for burns to his eyes.

An employee was changing a line hook-up for production to clean-in-place. After he tried to loosen a clamp, steam and water hit the right side of his body, causing second-degree burns on his torso, right arm, and thigh.

An employee was on a ladder to fix a steam header valve that was leaking. The employee was sprayed on the face/neck by hot condensate. He then jumped off the ladder and cut his leg.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

An employee was removing an elbow on the bottom of a tank to fix a clog when they were sprayed with hot water and a caustic chemical, resulting in burns to their chest and back.

An employee was unclogging the drain of a butter trolley that is used to transport churned butter to be packaged. His right middle finger contacted the auger at the bottom of the trolley resulting in amputation at the middle knuckle. The drain was not guarded at the time.

At 6:30 a.m. on July 1, 2022, an employee was helping tear down a pasteurization milk plate when hot water was discharged from a pipe. While moving to avoid the water, the employee fell, suffering a dislocated right shoulder as well as burns on her back from the water.

An employee was pushing material forward on a conveyor belt. The employee's left index finger was caught between the rollers and the belt resulting in amputation of the pad of the finger.

An employee was using a band to pull pallets of cans off a truck when the band snapped, causing the employee to fall backwards onto the floor. The employee suffered a fractured left arm and hand.

An employee tripped over a bin and fell to the floor. She landed on her hip and braced herself with her arm, resulting in a fractured hip and wrist.

An employee was diagnosing the lack of flow of product to a powder bin. The employee removed the rotary star valve below the bin. While he was reinstalling the valve, his right middle finger was crushed between its shaft and its housing. The finger was partially amputated.

An employee was standing on a multi-purpose ladder installing head flashing over a window. Their feet were approximately 6 feet off the ground and three rungs from the top. The employee lost their grip on the drill they were using and the ladder tipped over. The employee fell from the ladder to the ground, resulting in a fractured left wrist, bruised chest, and lacerations to their face and lip.

An employee was inspecting the overhead area of a single-person lift and repositioning the lift through a double doorway. As they were transitioning through the doorway, they were caught between the doorframe and the vertical mast of the lift. The employee was hospitalized with injuries to the nose, left maxillary sinus, left orbital bone, and left jaw, with fractures at the maxillary sinus area.

An employee was loading a 4-inch wooden door into a pickup truck when he felt a pop in his right side behind his shoulder blade. He was hospitalized later that day and underwent surgery, having suffered a collapsed right lung.
