An employee was using a pallet jack to move a pallet of doors when he bumped into a stack of mirrors on a pallet. As he attempted to stop the mirrors from falling, his right arm was lacerated by the sharp edges of a mirror.

An employee was using a miter saw to cut pine wood blocking stock. The wood stock was pulled into the sawblade, which amputated four fingers on the employee's left hand, also lacerating the thumb.
On November 01, 2017, at approximately 3:45 p.m., an employee was helping to unload materials at a receiving dock. The employee was next to a pallet of columns measuring approximately 10 feet long and approximately 6 feet high. A forklift operator was on the opposite side of the columns and manipulating them when the pallet and columns tipped over, causing the employee to fall between the trailer and a guard system. He hit the concrete approximately 4 feet below, fracturing his neck and back.
