An employee was walking beside a dump truck, flagging traffic. The truck backed up and ran him over. He suffered two broken ankles, a broken left thumb, a broken rib, and a punctured lung and was hospitalized.

An employee was connecting two concrete traffic barriers. As he was placing the securing pin between the two barrier pieces, one barrier shifted, pinching his hand between the two barriers. The employee sustained an amputation to the left middle fingertip.

A front end loader drove over a heavy steel plate (the panel/side of a trench box) that was lying on the ground. The contact caused the panel to strike and break an adjacent employee's leg.

An employee was helping to install ground pipe in a deep trench. He was outside of the trench box on top of the 60-inch, pre-cast pipe form and bracing pipe. A back hoe operator was attempting to move the trench box when a portion of it made contact with the employee's left foot, crushing and breaking it.

The line was undergoing the regen process when a piece of scaffold struck a PVC pipe that transported hydrochloric acid (HCl). The injured employee was placing material inside a scaffold rack when he was sprayed in the face with HCl and sustained chemical burns to his eyes, face, chest, right arm, and left leg.

Samsung E&C America, Inc.

An employee was moving a ramming machine to a staging area. While using the controls on the side of the machine, there was a sudden heavy rainfall causing the equipment to slide. The ramming machine tipped over and caught the employee against the ground. The employee sustained a fracture to the right side of their pelvis and was hospitalized.

United Dairy Farmers

The injured employee was training a new employee on a dough machine. She went to show where the batter comes from and her right index fingertip was cut. The employee sustained an amputation to the fingertip. The machine was not guarded at the time of the incident.

MVA CONSTRUCTION LLC

An employee was operating a roller machine on a dirt slope when the machine hit a bump and fell onto it's side. While trying to get free of the falling machine, the employee's legs were pinned by the top bar of the cage. The employee was hospitalized and his left leg was amputated. He also sustained fractures to a hand and finger.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.
