An employee was getting a tool bag that was on a second-story roof when they fell from the roof to the ground. The employee was hospitalized with injuries to their rib cage and lower back.

The bucket then tipped causing the employee to fall to the ground. He sustained fractures to his right elbow, right wrist, and left femur.

An employee was laying down felt paper on a new residential property when they fell approximately 21 feet to the ground. The employee sustained fractures of the L2 vertebra, right tibia, and left wrist as well as a left heel dislocation. The employee was wearing fall protection at the time.

An employee was moving a large package from a slide when her left arm got caught between the slide (tilt tray) and the package. She was hospitalized with a fractured left arm.

Parsec LLC

An employee was operating a switch truck at a rail yard. He got out of the switch truck to unlock a container that was attached to a unit in front of him. The switch truck rolled and pinned the employee against the container. The employee sustained a fractured pelvis, a ruptured spleen, and a possible spinal cord injury.

An employee was sidestepping along the catwalk of a railcar when he lost his balance and fell approximately 4 feet to the ground. The employee's right hip was fractured.

An employee was working to realign the two top sections of a set of collapsible rollers (trailer-on-flatcar rollers). The rollers suddenly fell into place and pinched her left index fingertip against the lower sections. She suffered a partial amputation to the fingertip.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

GXO Logistics

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
