The employee was hospitalized with contusions and lacerations to the neck, back, and shoulder.

An employee was in a manhole pulling a 120-volt spider to connect it. The spider snapped and struck his chest. He was shocked and hospitalized.

An employee was carrying a 4X4 in an alleyway when he tripped and fell, breaking his right lower leg.
