A temporary employee was installing electrical wire from a ladder when they missed a rung and fell 1-2 feet to the floor. The employee's elbow and knee were injured.

An employee was setting a column on a footing. A choker was around the column and forklift being used. The column began to slide, and the employee's right hand was caught between the choker and the column. The employee's middle and ring fingers were amputated at the middle knuckles.

An employee was cutting material with a circular saw. The saw cut the tips of the employee's fingers, causing an amputation and injuries requiring stitches.

A temporary employee slipped on ice and fell to the ground, suffering a leg injury.

An employee had placed a casting on a crane hook and was operating the crane by remote control. When the employee grabbed the casting to turn it, it fell off the hook and struck the employee's fingers. The employee suffered a partial amputation to the right ring finger, as well as lacerations to the right ring and middle fingers.

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

A temporary employee fell from an A-frame ladder to the floor about 5 to 6 feet below. He suffered fractures to his left hip and was hospitalized, requiring surgery.

A temporary employee was operating an electric pallet jack at a loading dock. While maneuvering the pallet jack into a trailer, the equipment loaded onto the pallet jack began shifting to the left. The employee's left foot was caught between a yellow guardrail and the side platform of the pallet jack, and his left ankle was crushed and fractured.

A temporary employee was cleaning a room and compressing the contents of a garbage bin using her right foot. As she pulled her leg out of the bin, she lost balance and fell to the ground, landing on her left hip. The employee sustained a fracture to her left hip and was hospitalized.

An employee was driving an electric pallet jack when it struck a rack. The employee was hospitalized with a crushed and lacerated right ankle.

A temporary employee was filling a sweeper with gas at a construction site. He was shocked by the jumper cables, which caused him to move his right hand into the sweeper's rotating fan blades. His middle fingertip was amputated.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
