An employee was cutting steel bars with a horizontal bandsaw when the blade amputated his right small finger.
An employee bumped into equipment forks and sustained a bruise to the lower leg on 8/21/15. The incident was reported to the employer on 8/25/15 when the employee sought treatment. The employee was hospitalized on or about 8/26/15 for the treatment of a hematoma.
