An employee was assisting with the assembly of a cross-over sub. He picked up a 158-pound cross-over sub to place on a stand. His right index finger became caught between the sub and the stand. His finger was crushed and amputated.

An employee was sanding a part after welding it. His right thumb was caught between the metal and the sanding belt and was amputated below the fingernail.

Three employees were moving a tool from the wash bay to two tool stands. The rear tool stand became caught on the lip of the floor and collapsed, causing the tool to fall off the stand. One of the employees attempted to catch the tool, resulting in it falling onto and amputating his left small finger.

On 05/04/2015, at approximately 12:45 PM, an employee was assembling tools to hammer down some parts in the shop area of the work facility. His hand slipped, and the tool struck his right hand, amputating the right little fingertip. He was hospitalized.

An employee was installing wood trim surrounding a skylight inside a home. While holding a piece of trim in it's desired location, his pneumatic nail gun discharged, sending a 3/4-inch pin nail into his left forearm.

An employee removed a safety valve on a trailer that unknowingly contained nitrogen gas. The gas released from the trailer and pushed the employee backward off a scaffold approximately 10 feet before they struck a garage door, resulting in a pelvic injury.

Their right hand was caught between a rolling ladder and the equipment, resulting in fractures to the index and middle fingers. They were hospitalized and the middle finger required surgery.

An employee was working to install a drive belt on a water pump. He was holding the belt on both sides of the pulley, working to route it over the top. As he pulled down with his right hand, the pulley rotated and the belt slid over it, pulling his left little finger between the pulley and the belt. He suffered an amputation to the fingertip as well as a fracture to the ring finger.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.
