A crew was attempting to hang a metal horse head on a pumpjack while the injured employee was sitting on a 12-inch beam (12 feet off ground). As the crew was lowering the head, it fell out of its saddle and smashed the employee's left arm, which became very swollen and possibly fractured.

A crew was coming out of a rig hole with tubing when the air backup on the tubing tongs slipped and bumped the injured employee, knocking him over a hand rail of the rig floor. He fell 6 to 7 feet head first, lacerating his forehead and breaking his left eye socket. He was hospitalized.

This caused the cart to tip forward and the die to slide off the cart. The die struck the employee's left foot and their second toe was amputated.

An employee was using a forklift to load pallets of product on a trailer. He exited the forklift to adjust a pallet by hand. The pallet fell on him, resulting in an injury to the left leg.

An employee was lifting a 500-pound steel counterweight off a pallet using a magnetic hold jib crane. The counterweight detached from the crane and the employee sustained a left foot/toe fracture that required surgery.

An employee was working to install a drive belt on a water pump. He was holding the belt on both sides of the pulley, working to route it over the top. As he pulled down with his right hand, the pulley rotated and the belt slid over it, pulling his left little finger between the pulley and the belt. He suffered an amputation to the fingertip as well as a fracture to the ring finger.
