An employee was on an 8-foot A-frame ladder, installing new flex-duct on an HVAC system. The ladder became unstable; it and the employee fell in different directions, and the employee landed on the tile floor. The employee suffered injuries to the left hip and both knees.

An employee was on a ladder, looking for a drill hole in an awning. The employee stepped on the awning and fell 12 feet onto the concrete dock below. The employee suffered fractures to the right arm, right leg, pelvis, and ribs.

On December 14, 2023, at 8:40 p.m., three employees were inspecting milling machines for startup. A few of the sleeves on the mill drum needed to be changed out. The injured employee was trying to input a sleeve on the mill drum using a hammer. The hammer hit the edge of the drum, which caused a metal piece to splinter off and strike the employee's cheek just below the left eye. The metal splinter embedded in the back of their eye socket, resulting in pain, swelling, and bleeding that required hospitalization.

An employee was pushing a line of carts into a vestibule when the door cable snapped and struck his right ankle. The employee suffered a fracture that required surgery and hospitalization.

Employees were conducting a snubbing operation. While making up the tubing swivel onto the joint in the mouse hole, employee 2 proceeded to pull power tongs off the pipe. At the same time, the injured employee went to re-adjust the tong heads. The make/brake handle was pulled and the injured employee's right hand got caught in the tongs. The injured employee's index, middle, and ring fingers were amputated and he required surgery.

On 9/26/2025, an employee was using a pair of scissors to cut a zip-tie that was securing a box of files. The scissors slipped and lacerated the employee s left wrist. The employee was hospitalized.

An employee had been taking fuel samples from the top of a barge. The employee was hospitalized with heat exhaustion.
