An employee was fixing a mechanical casket lift in a vehicle when the lift came down on their left-hand fingers, resulting in amputation of two fingers.

An employee was helping to remove the clamp attachment paddles from a forklift so they could then repair a hydraulic leak. The clamp fell and pinned the employee's right ring finger against the mast. The employee's finger was partially amputated.

An employee was on a truck attempting to stow an aluminum extension ladder onto the truck's conveyor boom system. As the conveyor mast was lowered, the boom control area came down on top of the employee's left foot, resulting in two toe fractures.

An employee was standing on the elevated forks of a forklift to install the fork chain anchor nut. The employee slipped and as he went to catch himself, he accidentally engaged the up-down control for the forks. This caused the forks to fall onto his foot. An employee was operating a side loader designed for narrow aisles to pick up pans filled with steel, which include 12-foot pans and others with 20-24 foot pans. The employee pulled the required amounts from the pans using a crane, and then packed and labeled the items for shipment to the loading dock. The side loader struck a pan, causing the cab to detach and land on the employee's leg, resulting in a fracture.

A delivery driver was helping a customer unload a strut. The banding around the strut broke loose and lacerated the employee's lower left leg. The employee was hospitalized and required stitches and surgery to repair the patellar tendon.

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

An employee was working in the food service warehouse when he experienced chest pain, difficulty breathing, and lost consciousness. The employee sustained carbon monoxide poisoning.
