On December 28, 2024, at 10:23 AM, the injured employee was spotting for the operator of a shear crane while conducting tree trimming/removal operations. The injured employee was standing about 4 feet from the ground on top of scrap steel trusses. After the shear crane cut the tree, it began dragging a large branch which struck the injured employee and knocked him down onto the scrap pile. He sustained four broken ribs on the left side and a left lung contusion. The employee was hospitalized.

On June 26, 2024, an employee was using a pry bar to flip an aluminum die. The pry bar slipped out and the die crushed the employee's right ring finger, which resulted in a partial fingertip amputation.

An employee was torch-cutting a coupler off a railcar. The coupler fell onto the employee's left foot, breaking it.

An employee was directing traffic and was struck from behind in the legs by an I-beam that was knocked over by a crane. Both of the employee's legs were fractured.

An employee was tossing a damaged toilet into a compactor. The toilet bounced off the compactor and lacerated the employee's right wrist, cutting an artery and tendon.

An employee was operating a hydraulic jack to bend a frame when the jack came loose and struck the employee. They were hospitalized for a spleen injury.

A truck had been towed into the shop with a flat tire and a blown airbag. An employee checked the tire for damage and was airing the tire. The employee was installing the valve core when the bead slipped off the rim. Air escaped, and the aluminum wheel struck the employee's head and face. The employee became unresponsive and was bleeding.

On December 14, 2023, at 8:40 p.m., three employees were inspecting milling machines for startup. A few of the sleeves on the mill drum needed to be changed out. The injured employee was trying to input a sleeve on the mill drum using a hammer. The hammer hit the edge of the drum, which caused a metal piece to splinter off and strike the employee's cheek just below the left eye. The metal splinter embedded in the back of their eye socket, resulting in pain, swelling, and bleeding that required hospitalization.

An employee was pushing a line of carts into a vestibule when the door cable snapped and struck his right ankle. The employee suffered a fracture that required surgery and hospitalization.

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

An employee was checking on a machine outside of the building and removed a cover to clean out a blocked area. His hand was caught by a rotary valve in a dust collection machine. The employee's fingertip was amputated.

A shear operator's hand and fingers were caught in a machine's point of operation. The operator's right index finger was crushed and was partially medically amputated.

An employee was operating a forklift. He parked on a ramp and got off the forklift to retrieve material, at which point the forklift rolled backward and ran over his leg. The leg was broken and he was hospitalized, requiring surgery.

An employee was using a shear to cut metal when the blade cut his left ring finger and amputated his left little finger.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.
