On 11/09/2017, an employee was operating a blow mold machine when it jammed. The employee was trying to unjam it when the machine crushed and lacerated the right index finger. On 11/21/2017, an employee was operating a capper machine when it jammed. She was trying to unjam it when her sleeve was caught on a moving part, pulling her right arm into the machine and fracturing it.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.
