An employee was feeding cardboard sheets into a machine when a jam occurred. He was attempting to unjam the machine when he tripped and used his right hand to catch himself. His right arm/elbow area was then caught between the machine's anvil roller and stop shaft before the machine fully stopped. He suffered a skin detachment injury to his right arm's elbow/bicep area, requiring hospitalization and surgery. The machine was not locked/tagged out at the time of the incident.

An employee was walking underneath a silo when he tripped. The employee reached for a head pulley gear box and his finger became caught in the belt, resulting in a degloving injury that required medical amputation of the right middle fingertip.

On July 18, 2022, an employee was carrying chains to hook to a tool building. The employee tripped, and the eye ring for the four-way chains came down onto the eye of the tool building, catching his right index finger and amputating it above the first joint.

An employee was moving a dumpster from inside an apartment to the outside when he stumbled and his ankle was pinned between the curb and the dumpster. He suffered a broken left ankle.

On October 22, 2025, an employee was working to put a conveyor chain back on the track of a conveyor when their fingers were caught between the chain and the chain track, pulling their right hand into the sprocket. Four fingers were amputated.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

McAneny Brothers, Inc.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
