An employee was dislodging a towbar and hitch between two golf carts. The upper part of the employee's left index finger was pinched in the towbar/hitch, causing an amputation at the last joint.
An employee was operating a forklift when it struck a large planter and rolled onto its side. The employee was hospitalized with a fractured left ankle.
Ventilation fans inside the restrooms of the employee cafeteria experienced a mechanical issue, causing smoke to occupy the restroom. An employee working in the cafeteria at the time of the mechanical issue experienced trouble breathing and had an asthma attack due to smoke inhalation, requiring hospitalization.
The injured employee was installing fan belts and alternators. Another employee came into the work area with a torque gun and began poking the injured employee with the tip of the torque gun and then activated the tool. The spinning tip of the torque gun caught the injured employee's left glove and amputated her middle finger.
