On December 30, 2017, at approximately 3:30 PM, an employee was attempting to clean clogged meat from a drain on a meat stuffer machine when a grinder unit in the drain caught his right index finger, amputating it to the first knuckle.

His fingers were in the holes to pull off the face plate when the machine actuated. His right middle finger got trapped between the face plate and the inner plate, resulting in amputation.
