An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.

An employee was using air to blow out the air wash to the blender and grinder station. As the employee reached to verify that the air wash was clean, the unit pinched and amputated their fingertip.

An employee was retrieving a lost drill pipe with a lifting bail when their left thumb got pinched between the table and handle of the lifting bail while trying to re-thread the pipe to lift it out. The employee suffered an amputation to the left thumb.

An employee was removing a core chuck from a stationary roll when their right ring finger was pinched between the chuck and the roll. The employee's fingertip was amputated.

An employee was helping to install a tarp on a roof when they fell through a skylight to the ground below, resulting in fractures to their face and ribs.

On August 5, 2025, an employee was exiting a room when his left middle and ring fingers became caught in the door as it shut. The middle fingertip was amputated.

An employee was driving a forklift and unloading racks of glass from a truck. A rack began to tilt; the employee exited the forklift and went to stabilize it. The rack tipped and pinned his left arm against the forklift. The forearm was broken and required surgery.

An employee was sitting on a stack of steel beams. When they stood up, the top beam fell onto the employee, resulting in a broken left leg.

An employee was working to tow a forklift with another forklift when the tow line came loose and struck their face. The employee sustained head, face, and eye injuries.

An employee slipped while climbing a ladder on a skidder, fell to the ground, and suffered a tibia/fibula fracture. The employee was hospitalized.

An employee was lowering the bucket of a track loader that contained a grade stake and the grade stake punctured his foot, resulting in hospitalization and surgery.

An employee was using a pike to move a pallet of sugar that was stuck in the powder racking. The employee fell backward, landing on his right leg. The employee's leg was fractured and he was hospitalized.

An employee was working to fix the forks on a forklift when the fork(s) fell on her toes. The second and third toes on her right foot were fractured, lacerated, and required stitches.

An employee was delivering a 65-foot boom lift to a rental customer's construction site. The employee set up the ramp truck in the parking lot adjacent to the construction site. He elevated the boom 4-5 feet to clear a temporary fence. While driving the boom lift off the ramp, the wheels slid to the driver's side of the ramp truck, ultimately sliding off the ramp. The boom lift fell off the side of the truck and rolled onto its side. The employee sustained a fractured clavicle, a dislocated shoulder, separated/fractured ribs, a punctured lung, and a fractured left femur. The employee required surgery.
