Employees were working to secure a low-hanging feeder. An employee was elevated in a bucket at a height of approximately 12 feet and was pulling the lashing wire. The lashing wire flipped up and struck the lower phase (120 volts) of power that was 2.4 feet above the feeder and strand. The employee sustained electrical burns to their chest, both hands, and the right forearm.

An employee was walking in a parking lot when he tripped over a bolt protruding from the ground and fell. He suffered two fractures in his right elbow, as well as a torn tendon leading to the elbow. He was hospitalized and required surgery.

An employee was on an extension ladder, installing cable from a pole in an alley. The employee fell from the ladder to the ground and suffered a broken left arm.

A technician arrived at a service call. He exited his vehicle and opened the passenger side door of the van to collect his face mask from the center console. A gust of wind caused the door to close on his left index finger amputating it at the first knuckle.

An employee was performing door-to-door sales. While walking between sales visits, the employee tripped on an uneven sidewalk/driveway slab of concrete and fell to the ground, striking his left knee area and suffering bone fractures.

An employee was going to the breakroom when they tripped over a chair and fell, resulting in a hip fracture.

An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.

An employee tripped over a box and fell to the floor, fracturing their right arm.

An employee was preparing for a CT scan when she tripped over a loose strap and fell to the floor, fracturing her hip.

An employee was troubleshooting the control setup for a variable air volume box above a ceiling. The employee's hand came into contact with an exposed, energized 277-volt wire that had loosened from its terminal on the box's assembly. He was shocked and fell off a ladder. His left hand passed across the ceiling grid and was lacerated. He also suffered an electrical burn to the right hand and was hospitalized.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was standing on an A-frame ladder approximately 6 feet above the floor while he attached a suspended light fixture to the ceiling. The employee lost balance, and fell to the tiled floor below, sustaining blunt force trauma to his head.

An employee was inspecting the overhead area of a single-person lift and repositioning the lift through a double doorway. As they were transitioning through the doorway, they were caught between the doorframe and the vertical mast of the lift. The employee was hospitalized with injuries to the nose, left maxillary sinus, left orbital bone, and left jaw, with fractures at the maxillary sinus area.

The injured employee was performing routine maintenance on a fan belt/pulley. The injured employee and another person were moving the pulley back and forth manually to find a bearing issue. The injured employee's left ring finger got caught between the belt and the pulley wheel, resulting in a soft tissue amputation.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
