An employee was riding the elevator down from the 5th floor. It stopped between the 1st and 2nd floors and would not move. He tried to get the elevator to move, worked his way out of the opening, and fell about 6 feet to the pavement below. He broke several ribs.

Petroleum Service Corporation

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

Absolics Inc.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

American Residential Products, Inc.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

Berkheimer Maintenance

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

High-Tec Industrial Services, Inc.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

An employee was releasing propane from an expired propane tank and a flash fire occurred, burning his chest, left arm, and left hand.

An employee was driving a reach truck when it struck another reach truck that just stopped with its forks raised. The forks penetrated the operator compartment of the employee's vehicle and broke his right tibia and fibula. He was hospitalized and underwent surgery.

An employee was separating lighting on electrical relays. The employee was shocked by wires and fell from a ladder to the ground, suffering a broken hip.

An employee was backing an ATV toward a loading dock when it rolled off the dock. The employee suffered a broken ankle and possibly cracked vertebrae.

At about 6:00 p.m., an employee was leaving his truck when he slipped and fell, his left side and back striking the truck's bumper. He suffered muscle and tendon injuries and lower back pain.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

Zimmerman & Herr

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

McAneny Brothers, Inc.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

Metz Culinary Management LLC

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
