An employee was operating a rotating tool to place thread protectors on the end of pipe. Her left hand was on the rotating part of the tool when the tool engaged, resulting in a left forearm fracture and a thumb laceration.

An employee was changing the blade of the B-line chop saw located in the PMC finishing area. As he tightened a nut with a wrench, the saw activated and the wrench was ejected from the machine striking the employee in the face. The employee sustained a fractured nose and facial lacerations.

An employee was walking through the yard to get his equipment and slipped and fell on the ice. The employee twisted his ankle and chipped a bone in his foot which required surgery.

An employee was walking behind the store with another associate to get paperwork from the delivery driver when she fell on the walkway and sustained a leg fracture that required surgery.

An employee fell while giving a skiing lesson and sustained a broken leg.

An employee was moving a sawed roll from the conveyor belt to the cart when the lifting magnet on the hoist gave way. The sawed roll dropped onto the employee's foot, resulting in a left metatarsal fracture.

An employee suffered a degloving of the little finger, which resulted in the amputation of the finger.

An employee was hooking a load up to a crane when the employee walked into a conveyor line. A steel pipe was coming down the conveyor line and struck the employee, causing side and stomach contusions.

On November 11, 2017, an employee suffered a hand fracture while attempting to stop a coil from falling off a horse.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
