An employee was moving a piece of an I-beam for welding when it rolled off the cribbing. The employee went to catch it when it fell and crushed the employee's finger, resulting in a partial amputation.

An employee was helping to disassemble large reels used to store steel wire when a 264-pound flange from the reel fell over onto the employee's left leg. The employee sustained multiple fractures to their leg and ligament injuries to the lower leg.

The injured employee was putting away materials on the warehouse floor as a co-worker was pushing a pallet onto a nearby shelf. The pallet knocked another pallet forward, causing it to fall onto the injured employee's shoulders. The employee suffered fractures in their right and left knees and ankles.

Employees were moving a single man lift into a building and reclining the lift to position it to fit through the door. The lift shifted and fell, causing the employee to sustain fractures to the left tibia and fibula. The employee was hospitalized and had surgery.

An employee was moving a light tower so it could be hitched to a truck. He grabbed the tongue of the trailer hitch on the light tower to slide it to the left. The tongue jack fell off, causing the light tower tongue to drop on the employee's right hand. The employee's middle fingertip was amputated.

An employee was walking down a hallway when they stepped on a small puddle of liquid and fell backward onto their left leg, resulting in a broken left femur.

An employee was on a 10-foot ladder replacing a fan motor on an HVAC system. While removing the guard to facilitate maintenance, the guard slipped. As the employee tired to grab it, their left hand fingers were caught between the belt and pulley resulting in a partial amputation of the little finger and fractures and lacerations to the middle and ring fingers.

An employee was using a fire escape when he slipped on the stairs that were wet due to rain. The employee fell down several stairs, hit his head, and injured his right hand.

Three employees were working on a steam pipe. They had depressurized the line and cut it. As they were preparing to replace the broken pipe section, one of them was struck by pressurized hot water and/or steam, which burned his right arm, chest, and back. He was hospitalized.

An employee was pushing a cart when they stopped to assist someone. As they turned around, they fell backward down the stairs with a box and hand truck landing on top of him. The employee was hospitalized and sustained rib fractures, a hematoma, and a compression fracture/spinal injury.

On September 22, 2025, an employee was making a delivery to a home. When walking up the sidewalk to the home, the employee tripped, fell to the ground, and hit her head and face against the concrete. The employee was hospitalized due to a laceration to the face and a fractured orbital socket.

At about 10:30 a.m. on April 9, 2025, a mail carrier was crossing between homes when a dog jumped on them and knocked them down. The employee's head hit the concrete ground; the employee suffered a head injury and was hospitalized.

A mail carrier slipped on an icy surface and fell, suffering a broken right hip.

On January 30, 2025, at around 8:45 AM, an employee was in the parking lot loading their vehicle with mail deliveries for the day when they slipped and fell. The employee landed on their arms and knees, resulting in injuries that required hospitalization.

At 11:26 a.m., on January 23, 2025, an employee slipped and fell on ice and injured her right ankle. The employee was hospitalized.
