An employee was cutting a banded pack of 1-inch thick lumber boards. Immediately after cutting the band, the uneven pile of lumber fell toward him and knocked him over. His middle fingertip became trapped between two of the boards, resulting in a fingertip amputation.
An employee was operating a trimming machine. The employee placed his foot on the machine in order to activate the photo eye and his pant leg got caught in the machine, causing a laceration to his right heel.
An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.
