On September 9, 2022, an employee was lifting a bag of linen when he lost his grip and fell backward, striking his back on a trash bin. The employee experienced pain in his back and subsequent numbness in his legs, resulting in hospitalization.

An employee was loading a box truck parked at a loading dock. A ratchet strap that he was using came unhooked, causing him to fall backward through a gap between the truck and the building. He landed on his back on the asphalt below and suffered a back injury, as well as a head laceration.

An employee fell on ice in the parking lot and was injured, requiring hospitalization.

An employee was looking at a hydraulic lift that was stuck when the lift fell and struck the employee, causing a back injury.

An employee was standing on a ladder to remove an obstruction from a conveyor when the employee slipped and fell off the ladder, striking the conveyor before striking the ground. The employee suffered a broken rib, bruised eye, and finger injury.

An employee was helping to disassemble large reels used to store steel wire when a 264-pound flange from the reel fell over onto the employee's left leg. The employee sustained multiple fractures to their leg and ligament injuries to the lower leg.

The injured employee was putting away materials on the warehouse floor as a co-worker was pushing a pallet onto a nearby shelf. The pallet knocked another pallet forward, causing it to fall onto the injured employee's shoulders. The employee suffered fractures in their right and left knees and ankles.

Employees were moving a single man lift into a building and reclining the lift to position it to fit through the door. The lift shifted and fell, causing the employee to sustain fractures to the left tibia and fibula. The employee was hospitalized and had surgery.

An employee was moving a light tower so it could be hitched to a truck. He grabbed the tongue of the trailer hitch on the light tower to slide it to the left. The tongue jack fell off, causing the light tower tongue to drop on the employee's right hand. The employee's middle fingertip was amputated.

A maintenance technician was investigating a noise coming from a conveyor. His left hand was caught by a roller and pulled in up to the forearm. The employee's forearm was fractured.

An employee was preparing a truck at a loading dock. She was placing the manual dock leveler device when her left middle finger became caught in the leveler. The employee's finger was partially amputated.

An employee was using a machine that irons and folds table napkins when a string from her sweatshirt got caught in the rollers. Her hand was pulled into the machine's point of operation, resulting in crush injuries that included a broken wrist.

On March 20, 2025, an employee was making a delivery at a customer location when they slipped on ice and fell off the bumper of the delivery truck to the ground. The employee was hospitalized with a head injury.

An employee was manually loading a washing machine when her foot became caught under an air hose and she fell to the concrete floor. The employee sustained a fractured left hip and required surgery.

An employee slipped while climbing a ladder on a skidder, fell to the ground, and suffered a tibia/fibula fracture. The employee was hospitalized.

An employee was working to fix the forks on a forklift when the fork(s) fell on her toes. The second and third toes on her right foot were fractured, lacerated, and required stitches.

An employee was delivering a 65-foot boom lift to a rental customer's construction site. The employee set up the ramp truck in the parking lot adjacent to the construction site. He elevated the boom 4-5 feet to clear a temporary fence. While driving the boom lift off the ramp, the wheels slid to the driver's side of the ramp truck, ultimately sliding off the ramp. The boom lift fell off the side of the truck and rolled onto its side. The employee sustained a fractured clavicle, a dislocated shoulder, separated/fractured ribs, a punctured lung, and a fractured left femur. The employee required surgery.
