On April 28, 2018, an employee was guiding a forklift operator when the forklift lowered a crate onto his foot, crushing his right big toe. His toe was later amputated.
An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool.
