An employee was trying to transfer lumber from a bundle on a forklift into the bed of a truck. The lumber was frozen together, and another employee began to tap it with a hammer. The wood began to slide, and the first employee's hand was caught between the falling wood and a stack of wood. The employee's right ring fingertip was amputated.

An employee was getting a tool bag that was on a second-story roof when they fell from the roof to the ground. The employee was hospitalized with injuries to their rib cage and lower back.

An employee was installing a clip on the roof when they slipped and fell approximately 25 feet to the ground below, resulting in a broken hip, broken leg, and bleeding in the brain.

An employee was setting up the worksite for the day. While moving an extension cord, he fell backward off the roof of the two-story building and sustained a broken pelvis.

An employee was going up in a lift to begin trimming trees with a chainsaw. He was approximately 20-25 feet in the air when the latch to the bucket got caught and detached. The bucket then tipped causing the employee to fall to the ground. He sustained fractures to his right elbow, right wrist, and left femur.

An employee was laying down felt paper on a new residential property when they fell approximately 21 feet to the ground. The employee sustained fractures of the L2 vertebra, right tibia, and left wrist as well as a left heel dislocation. The employee was wearing fall protection at the time.

An employee was fixing a broken purlin at a worksite when they fell from the framing to the concrete floor approximately 19 feet below. The employee sustained brain bleeding and fractures to their face, vertebrae, and wrists.

Employees were removing scaffolding that was used to repair an ion exchange tank. The line was undergoing the regen process when a piece of scaffold struck a PVC pipe that transported hydrochloric acid (HCl). The injured employee was placing material inside a scaffold rack when he was sprayed in the face with HCl and sustained chemical burns to his eyes, face, chest, right arm, and left leg.

An employee was moving a ramming machine to a staging area. While using the controls on the side of the machine, there was a sudden heavy rainfall causing the equipment to slide. The ramming machine tipped over and caught the employee against the ground. The employee sustained a fracture to the right side of their pelvis and was hospitalized.

The injured employee was training a new employee on a dough machine. She went to show where the batter comes from and her right index fingertip was cut. The employee sustained an amputation to the fingertip. The machine was not guarded at the time of the incident.

An employee was operating a roller machine on a dirt slope when the machine hit a bump and fell onto it's side. While trying to get free of the falling machine, the employee's legs were pinned by the top bar of the cage. The employee was hospitalized and his left leg was amputated. He also sustained fractures to a hand and finger.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
