An employee was attempting to retrieve paper that had fallen into a labeler machine when the employee contacted the timing belt, amputating the left ring fingertip. The machine was running and guarded at the time of the incident.
On April 9, 2025, an employee was cleaning anilox rollers in a machine with a rag. The machine caught the rag and then the employee's left hand, resulting in amputation of the middle finger.
On December 17, 2016, at 7:30 a.m., an employee was jogging a machine while attempting to adjust a poly wrapper when a plate came down and amputated his left middle fingertip.
