An employee was oiling a conveyor chain when his right index and middle fingers were caught between the chain and sprocket, resulting in amputations.
An employee was installing a horizontal rack beam from a scissor lift. When he moved the scissor lift, he was pinned against the rail of the lift and the rack beam. He then fell from the lift. He suffered fractured vertebrae and a severed lymph node system.
