An employee was participating in volleyball warm-up practice. The employee was serving volleyballs overhead when he suffered a hernia and was hospitalized.

On February 15, 2021, between 4:30 and 5:00 p.m., a TCU Facilities employee was changing out some stained ceiling tiles in an office. He was standing on the fourth rung of an 8-foot fiberglass stepladder, removing the tile, while a coworker on the floor took the old tile and handed him the new tile. He had to disconnect the ceiling grid to remove a tile. It came loose suddenly and his right foot slipped off the ladder. His coworker caught him, but his left foot got caught in the steps of the ladder. He suffered a dislocated left ankle and was hospitalized, undergoing surgery.

A crew was using a seamer to install roof panels on a new building. As the employee was placing the seamer on a platform at the edge of the roof, the employee stepped onto the platform. The platform and the employee then fell approximately 23 feet to the ground. The employee sustained a lower back injury.

An employee was moving 6x6 wood debris while repairing a roof when the roof caved in and the employee fell 24 feet to the concrete floor below. The employee suffered bruised lungs, a scratch on his right lung, multiple spinal vertebrae fractures, and a fractured left iliac crest. Fall protection was not used at the time of incident.

An employee was climbing a 32-foot fiberglass extension ladder to get onto a roof. When the employee was a little more than halfway up the ladder, the fiberglass cracked and the ladder failed. The employee fell onto the deck below, resulting in fractures to both heels.

An employee was inspecting the existing metal roofing system of a commercial property when the roof gave way and he fell approximately 20-25 feet to the floor in the warehouse space below. The employee sustained a head injury, kidney and liver lacerations, and fractures to the orbital bone, right wrist, pelvis, and ribs.

An employee was using a portable ladder to descend into a shaft. Both side rails of the portable ladder snapped, causing the employee to fall approximately 22 feet to the concrete level below. The employee sustained lacerations on their head and multiple fractures to their hip and pelvis.

An employee was removing boxes of filters from a skid when he tripped on a manual pallet jack. The employee fell and struck his back on the base of the pallet jack, resulting in hospitalization with fractured ribs.

An employee was walking through a parking garage. She tripped over a curb and fell forward onto her right knee, then her left knee, both hands, and her face. She suffered an injury to the left knee that required surgery, as well as scrapes on the chin and both palms and soreness in the left wrist.

An employee was using a hose to clear storm drains after a sewer backup. The employee fell backward and his head struck the curb, resulting in an Injury on the right side of his head, generalized bruising, and pain in his left ankle, right hip, left shoulder, and neck. The employee was hospitalized.

An employee was working on an exhaust fan on the roof when he tripped on a wire and fell to the ground. The employee sustained scrapes on his elbow, and a fractured right knee with a torn ACL. The employee was hospitalized.

An employee was replacing the back bracket on a deep fryer when her screwdriver slipped, causing her to fall forward into hot oil. She suffered burns on both arms up to the elbows (including the hands and fingers), as well as on the face and neck. She was hospitalized.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.

An employee was removing plastic material from a production line when the machine cycled and amputated his left index, ring, and little fingers.

An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.
