Final narrative
An employee was pulling a pipe from a rack using a tool when the tool slipped, causing her to lose her balance in a backwards motion. While in motion, her foot became caught on a rail (extends approximately 1/2-inch above the surface), causing her to fall on and break her right hip in two places. She was hospitalized.

An employee was fitting a welding key into a slot to join two pieces of iron that were on a work table. The key did not fit properly so he removed it to investigate. His finger was in the key slot when one of the pieces of iron shifted, resulting in the amputation his left middle fingertip.
An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.
On December 27, 2023, an employee was delivering mail to an apartment complex when their knee gave out and they fell to the ground. The employee was hospitalized with a left tibia fracture.
