An employee was installing a receptacle on a live floor box. He was extending the floor box using PVC glue to secure it. The flammable glue ignited with the live wire in the floor box. The employee's left hand sustained second degree burns.

Two employees were pulling wire through a drop ceiling. The wire got stuck on something in the ceiling, then came loose as one of the employees was pulling on it. The slack caused him to lose his balance, and he grabbed the metal framing of the drop ceiling to avoid falling.
